We have a lot to discuss in this week’s episode, so with that being said, here are the major events that went down.
Peaky Blinders Season 6 Episode 4 Recap:

The unfateful demise of Tommy and Lizzie’s Daughter, Ruby.
Episode 4 kicked off with Tommy’s daughter, little Ruby, fighting for her life against TB. She is readmitted to the hospital, and her condition looks bleak. She tried her best to come back alive; however, it was a battle that proved too much for her.
Incredibly grief-stricken, Tommy and Lizzie did their traditional rites and bade her farewell for the afterlife. Tommy doesn’t seem to be in his right mind as he still thinks Ruby’s death happened because of a curse.
The curse was set on Ruby after Tommy’s business with the Russian sapphire, which eventually led to Connie Barnwell’s death.
An enraged Tommy, engulfed with the motive of vengeance, seeks out the Barwell camp and brings hellfire upon them. So far, season 6 has been pretty violent, and Tommy’s short-sightedness isn’t helping their current situation.
He is dragging himself and the others to a mess that seem likely to be avoided. But that’s how our Tommy is.
On the other hand, Lizzie isn’t impressed by Tommy’s actions; however, Tommy indulges himself with Arthur in a heart-touching scene. He also touched liquor for the first time since Polly’s death.
Tommy has a second child?
Meanwhile, Peaky Blinders Season 6 Episode 4 is far from over, as we still have two significant revelations to take place. It seems Ruby isn’t the only child of Thomas Shelby.
Esme reveals that Tommy has an illegitimate son named Duke. The son she talked about was conceived before the war as she says, “A daughter lost, a son found,” conceived “under a hazel tree in 1914.”
As the episode progressed, it shed light on the Fascist takedown that Tommy had his eyes on. Tommy is in no mood of stopping here and is fully driven to end what they have started.
Tommy and Gina’s Deal
Tommy also caught up on Gina’s dirt and used that to his advantage. Tommy had his eyes on Gina, and after catching her little secret with Mosley, he threatened to reveal it to her uncle.
Gina had a deal with Tommy to sweep it under the rug in exchange for becoming his spy in the Berlin meeting set between the German officer and her uncle.
Peaky Blinders Season 6 Episode 4 Ending Explained: Tommy Suffering from TB
Peaky Blinders Season 6 Episode 4 ended with delivering bad news for Tommy. As we saw, Tommy had been ignoring the calls from his doctor; it’s now that we know what the deal. The doctor had to be the bearer of the bad news that Tommy was dying from TB.
He also reveals that Tommy has a meagre year to 18 months to live. Hear this, Tommy prays for strength and time to get his work done before his demise.
